refactor: 优化 TraceContext 追踪系统，支持安全嵌套并保留元数据

- 实现 ContextVar Token 机制，确保嵌套 Trace 的正确退栈
- 为 with_trace 装饰器添加 @functools.wraps 以保留函数原始元数据
- 移除初始化错误的 _token_stack 变量
- 增强 get_or_create 语义，支持自动绑定当前上下文
